Age of the child as on 31.03.2021 should be 3+ years (i.e born between
01.04.2017 and 31.03.2018).
Please visit the school website before filling up the form to get detailed
information about the school.
Incomplete or incorrect application will be rejected.
All the fields marked with * are mandatory.
The payment mode is ONLINE only.
Please retain the Transaction Id/Application No and the Copy of the form
received through the mail, for any further correspondence.
Children residing beyond 6 kms shall be admitted only in case vacancies
remain unfilled even after considering all the students within 6 kms.
The distance will be measured from school using google maps aerial
distance. (Sample attached).Any discrepancy in the distance will lead to
cancellation of the form.
Proof of Residence should be considered as any of the following
documents: (Please note that the residence address should be same as in
the below mentioned documents. These documents MUST be available
with the parent at the time of verification of documents, if selected.)
Regarding documents valid as proof of address. Some of indicative
documents which can be considered as proof of residence of parents/child:
Ration Card/Smart Card issued in the name of parents
(Mother/Father having name of child)
Domicile certificate of child or of his/her parents.
Voter I-Card (EPIC) of any of the parents.
Electricity bill/MTNL telephone bill/Water bill/ Passport in the name
of any of the parents or child.
Aadhaar Card/UID card issued in the name of any of the parents.
No documents to be submitted in the school after filling the online form.
No photographs to be uploaded while filling the online form.
NO EDITING IS POSSIBLE in the FORM after submission.
In case of any error in the form, follow the steps as given below:
1. Fill a NEW FORM by making a fresh payment.
2. A new registration number will be generated.
DATE OF BIRTH CERTIFICATE of the child: It must have the name of the
child and the parents as submitted in the registration form.
